Gambaran CBCT Impaksi Gigi 12 dan 13 akibat Complex Odontoma

Odontoma merupakan tumor odontogenik jinak yang paling sering ditemukan di rongga mulut, dengan prevalensi mencapai 21-67% dari seluruh kasus tumor odontogenik. Lesi ini umumnya bersifat asimtomatik, tumbuh lambat, dan sering terdeteksi secara insidental pada pemeriksaan radiografik. Odontoma terbagi menjadi dua tipe utama, yaitu compound odontoma dan complex odontoma, yang keduanya dapat menyebabkan hambatan erupsi gigi permanen, impaksi, serta persistensi gigi sulung. Meskipun radiografi panoramik merupakan modalitas diagnostik awal yang umum digunakan, keterbatasannya dalam visualisasi tiga dimensi dan hubungan anatomi seringkali menyulitkan penegakan diagnosis yang akurat, terutama pada kasus dengan lokasi lesi yang kompleks di regio anterior maksila. Penelitian ini bertujuan untuk mendeskripsikan gambaran embedded gigi 12 dan 13 yang disebabkan oleh odontoma complex pada pemeriksaan Cone Beam Computed Tomography (CBCT) 3D. Penelitian ini merupakan studi kasus deskriptif pada seorang pasien laki-laki berusia 15 tahun yang datang ke Departemen Bedah Mulut Rumah Sakit Gigi dan Mulut Soelastri, Surakarta, dengan keluhan gigi depan rahang atas kanan belum erupsi. Pasien menjalani pemeriksaan klinis ekstraoral dan intraoral, kemudian dirujuk ke bagian radiologi untuk pemeriksaan CBCT 3D. Data CBCT dianalisis secara visual pada tiga bidang ortogonal (coronal, sagittal, dan axial) serta rekonstruksi Multi-Planar Reconstruction (MPR) untuk mengevaluasi lokasi, ukuran, dan hubungan lesi dengan struktur anatomi sekitarnya. Pemeriksaan CBCT menunjukkan adanya massa radiopak ireguler dengan batas tegas yang dikelilingi halo radiolusen tipis, yang merupakan gambaran khas complex odontoma, terletak di sekitar gigi 12 dan 13. Gigi 12 dan 13 tampak mengalami impaksi dengan posisi yang tidak normal, di mana apeks akar kedua gigi tersebut bersinggungan dengan kavum nasal dan sinus maksilaris. Gigi 53 masih persistensi, dan odontoma berukuran 13,2 mm × 8,5 mm hingga 13,2 mm × 9,6 mm terdeteksi di sekitar area tersebut. Temuan spesifik menunjukkan adanya dilaserasi akar gigi 13 ke arah inferior serta kedekatan mahkota gigi 12 dengan kanalis insisivus, yang mengindikasikan potensi risiko komplikasi bedah. Kesimpulan: CBCT 3D merupakan modalitas pencitraan yang efektif dan direkomendasikan sebagai standar emas dalam mendeteksi serta mengevaluasi complex odontoma sebagai penyebab impaksi gigi insisivus maksila. Kemampuan CBCT dalam memberikan visualisasi tiga dimensi yang detail, terutama dalam menggambarkan hubungan lesi dengan struktur anatomi kritis, sangat bermanfaat untuk perencanaan perawatan bedah yang aman dan presisi. Penelitian lebih lanjut dengan sampel yang lebih besar diperlukan untuk mengkaji secara kuantitatif karakteristik radiografik odontoma kompleks serta luaran klinis jangka panjang pasca-pembedahan.
