Perbandingan Uji Diagnostik Kuesioner Sindrom Terowongan Karpal Boston dengan Kuesioner Kamath dan Stothard Pada Pasien Sindrom Terowongan Karpal di Rumah Sakit Adam Malik Medan

Latar Belakang: Diagnosis Sindrom Terowongan Karpal (STK) yang akurat sangat penting untuk terapi yang tepat sehingga dapat meningkatkan kualitas hidup pasien. Namun, tidak semua fasilitas kesehatan memiliki alat pemeriksaan konduksi saraf serta biaya pemeriksaan yang tidak sedikit sehingga banyak kasus tidak dapat terdiagnosis dengan baik. Dengan adanya keterbatasan tersebut maka dikembangkan kuesioner atau instrumen yang dapat membantu klinisi untuk mendiagnosis STK. Tujuan: Mengetahui nilai uji diagnostik Kuesioner Sindrom Terowongan Karpal Boston (KSTK-B) dengan Kuesioner Kamath dan Stothard (KKS) di RS Adam Malik Medan. Metode Penelitian: Penelitian ini menggunakan desain analitik observasional pada 51 pasien (67 manus yang diperiksa) yang mengalami gejala klinis STK di RS Adam Malik Medan yang memenuhi kriteria penelitian serta dikonfirmasi dengan pemeriksaan konduksi saraf. Kemudian dilakukan analisis statistik untuk menilai sensitivitas, spesifisitas, positive predictive value, negative predictive value, positive likelihood ratio, negative likelihood ratio, akurasi, serta nilai AUC dan ROC. Hasil: Terdapat 51 subjek penelitian, dengan mayoritas berjenis kelamin perempuan (66,7%). Hasil uji diagnostik untuk KSTK-B didapatkan sensitivitas sebesar 83,6%, spesifisitas sebesar 66,7%, PPV sebesar 92,0%, NPV sebesar 47,0%, LR+ sebesar 2,5, LR- sebesar 0,25, akurasi sebesar 80,5%, AUC sebesar 0,752. Sedangkan pada KKS didapatkan sensitivitas sebesar 87,2%, spesifisitas sebesar 75,0%, PPV sebesar 94,1%, NPV sebesar 56,2%, LR+ sebesar 3,49, LR- sebesar 0,17, akurasi sebesar 85,1%, dan nilai AUC sebesar 0,811. Kesimpulan: Pada hasil penelitian ini, KKS memiliki sensitifitas, spesifisitas, PPV, NPV, LR+, LR-, Akurasi, Nilai AUC, dan ROC yang lebih baik dibandingkan dengan KSTK-B dalam mendiagnosis Sindrom Terowongan Karpal.
